Lactation Support
I am an IBCLC with The Lactation Network, which accepts most Blue Cross PPO, Blue Shield PPO, Anthem PPO, Cigna PPO, Humana PPO, and VA CCN (Veterans Affairs Community Care Network) plans as well as most plans that utilize MultiPlan and PNOA. If your card includes the PNOA or MultiPlan logos, TLN may be able to take them.
Most people are eligible for at least 6 lactation visits 100% covered! (no copay or out-of-pocket). This includes a prenatal consult (which I highly recommend) and lactation visits, which can be in-person at home or Telehealth.
After insurance approval has been verified, we will be paired and both receive an email to confirm coverage. At that point we can coordinate to schedule your appointment.
If insurance cannot be pre-approved, consultations are to be paid at the time services are received. After care is completed I will provide you with a SuperBill to submit to your insurance for reimbursement.
Postpartum Care
As a licensed midwife I am considered an 'Out-of-network Provider'. PPO policies will often cover a percentage of my postpartum services, depending on your plan, after your annual deductibles have been met. Some private insurer plans negotiated by larger employers pay at 90-100%. Unfortunately most HMO's, like Kaiser Permanente, will not reimburse for care I provide.
Postpartum Care Package rates are sliding scale and may be paid for in advance or at appointments, however any outstanding balances need to be paid in full by the final postpartum visit.
Individual postpartum home visits are to be paid at the time services are received.
After care is completed I will provide you with a SuperBill to submit to your insurance for reimbursement.
